After the initial enrollment fee, the monthly fee will be billed in order to access the benefits of this program. You may cancel at any time with written notice prior to the end of your plan year. Please note: This is a 1 year contract that will continue indefinitely each month after 12 months until cancelled. If you cancel within the 1st year of plan enrollment you may be responsible for cost of services rendered that exceed total amount of monthly fees paid. Please feel free to call with any questions!
